	Eubie Blake Social Event Reservation Request Form...
	A NONREFUNDABLE deposit is required for all events. The event is not considered confirmed until the deposit is received. A refundable maintenance deposit is also required: however , may be retained to cover janitorial services.
	**Capacity 150 Seated**
	All applicants applying for use of the facility must be 25 years of age. Pets other than seeing eye dogs are not permitted.
	SUBMIT CATERERS COPY OF INSURANCE *SUBMIT ALCOHOL COPY OF INSURANCE
	Sale of alcohol beverages weather by the drink or for donation require a temporary license from the Baltimore City Liquor Board in addition to a certificate of insurance.
	The use of open flame candles or open flames is prohibited . Sound levels must be in accordance with the City of Baltimore. Children 12 years old and younger must be supervised by an adult.
